DCHC Supporting My Care Bubble Admission and Discharge Pilot

Devon Care Homes Collaborative (DCHC) is supporting My Care Bubble in the rollout of a new admission and discharge pilot in partnership with Derriford Hospital. The pilot focuses on improving communication, coordination, and continuity of care for residents moving between care homes and hospital settings.

My Care Bubble is a secure digital platform that supports clearer and more timely information sharing between care homes and hospital teams, particularly at key transition points such as admission and discharge. By improving the flow of information, the pilot aims to support safer discharges back to care homes and reduce avoidable delays or readmissions.

Through its support of this pilot, DCHC is helping to explore how digital solutions can strengthen partnership working between care homes and acute services, and improve outcomes for residents and care teams.
