Your Experience Matters: Informing the Single Patient Record Programme

The 10 Year Plan has promised us a Single Patient Record (SPR) in 2028.

'The SPR will bring together all a patient’s medical records into one place. Clinicians will be able to securely access it in order to deliver higher quality care - and patients will be able to add their own data from clinically validated wearables. The SPR will operate as a patient passport, making sure patients get seamless care no matter where they are in the NHS.'

This means:

  • Patients and carers will be able to contribute to their own personalised care plans.
  • Less duplication.
  • Health and care information will be accessible in one place.
  • MDT working will be much better supported digitally.
  • Referral processes will be able to be radically improved.

All significant issues from across the South West that we picked up on our recent Digital Frailty Discovery.

We have an opportunity to help shape the SPR.  If we don't have a say, we won't be able to complain if it doesn't deliver!

Here is a link to the questionnaire.

Can I ask you to fill this out and forward onto your teams. This is too big an opportunity to miss!
