Advance Care Planning using a brief intervention

A FREE learning event to be held in Torquay. Designed for health & social care professionals working at all levels.

Learning Objectives:

• To be able to know how to access the 'Planning for the future hub' and share the site with patients and families.
• To be able to identify and demonstrate the communications skills that facilitate a sensitive Advance Care Planning Brief Intervention (ACP BI) conversation.
• To demonstrate the use of the ACP BI in triads.

Prerequisites:

Prior knowledge and training about ACP is essential, e.g. attendance at our 'Advance Care Planning - what matters most?' learning event.

Places are limited and restricted to two free places per workplace. Additional places can be booked and will incur a charge of £25 per person.
