Personalised Care Workshop

Devon ICS is developing and implementing our plan for Neighbourhood Health, in the first instance supporting the 7% of our population with the most complex health and social care needs associated with up to 46% of hospital costs. This is across all ages and includes mental and physical health and will expand beyond the most complex 7% in year 2 of this work.
We will initially focus on people where there is greatest potential to improve independence, outcomes, freeing up resources to focus on prevention. It’s essential we do this with a personalisation and continuity of care approach that includes improving self-agency.
Personalised care is by no means a new concept and there is a huge amount of expertise, knowledge and examples of great practice across the county. We would like the opportunity to bring all of this thinking into one place so that together, we can start to explore what a joined-up approach to Personalised Care and Support Planning in Devon could look like and what we need to be able to bring it to life.
Neighbourhood Health Guidelines
We are planning 2 workshops, one on the morning of 22nd July and a follow-up in the autumn that will enable us to connect in our LCP systems, review personalised care in a neighbourhood context and take action on what we need to deliver this. We are very lucky to have Alf Collins to facilitate. Alf’s biography is attached for your interest.
To support a neighbourhood approach to the workshops, we plan to have opportunities to meet in person in the One Northern Devon area, Aperture for One Eastern Devon, at the Annex on the Torbay Hospital site for South and Crownhill Court for Plymouth, all connected via Teams.

Community physical and mental health services
Those working in a multi-disciplinary way with people with complex health and care or social needs, or who require help from multiple organisations
Integrated intermediate care – step up and step down between acute and community.
Urgent community response/hospital at home
Those overseeing support of this work in the voluntary, community, charity, faith and social enterprise (VCFSE) sectors.
